Note: Child must be 5 years old on or before September 1st to start Kindergarten.

  • Birth Certificate

  • Florida Immunization Record (out of state must be transferred to FL record)

  • Physical (Can not be over 1 yr. old as of Aug. 10th, 2021)

  • Social Security Card (optional)

  • Proof of Residency (Utility Bill, Mortgage, Rent or Lease Agreement)

  • Parent and/or Guardian's driver's license or military ID

  • Student's last report card and/or transcripts from prior school

  • Current Individual Education Plan (IEP) or 504 plan if student is ESE student, gifted, SLD, speech, language, etc.

  • Any legal documents that effect custody of student such as court order, Power of Attorney, etc.

My vision is that our school provides our students the opportunity to build close relationships with their teachers, staff, and peer group. Our school honors equity for all students and an atmosphere of high standards, met through a culture of love and support. I want to equip our students to effectively collaborate using communication and problem solving skills to aid them in thinking independently and the ability to navigate any challenge they face in life.

Reading Scholarship Accounts are available for students in grades 3 through 5 who are enrolled in a Florida public school and scored below a Level 3 on the grade 3 or grade 4 statewide, standardized English Language Arts assessment in the prior school year. For more information, click on the document button below.

Attention 3rd through 5th Grade Students:

During school season children may be exposed to head lice.  We would encourage parents to frequently assess their child for signs and symptoms of this pesky bug.  If you have concerns that your child may have head lice, you may speak to our school Health Tech.  She will be happy to provide assistance and/or provide information regarding head lice treatment.
